How to Use Public Forms and Check the Status of Your Ticket

Overview:

Most services require you to have an active WVU Login account to submit request forms. There are some public forms, however, that do not require an active Login (for example, forms for parents).

How do I complete a public form?

  1. Read through the service description.
  2. Click on the top blue button on the page.
  3. Complete the form.
  4. Make sure the name and contact information fields are correct, so we may contact you to provide assistance.
    Note: If you are signed into the site, you will not see the contact information fields.
  5. Click Request to submit the form.

Have a Login account? If you have an active Login account, sign in first. Signing in will ensure that the ticket is associated to your account.

How do I check the status of my ticket?

Open the email you received after you submitted the request form. The subject line will have the ticket number and title (Example: Service Request #1122334). The email will be sent from Your Name followed by <notify@teamdynamixapp.com>.

If you do not have an active Login account, reply to the email and ask for an update.

If you have an active Login account, you can click on the link in the email to view the status of your ticket. You may be prompted to sign into this site.
